A major reason for the failure of planning is its starting point. We always set the starting point to a new day. Till that day, we purposefully waste the time, thinking that there is still enough time to start the plan as planned. At this point, it is sure that the planner only wanted to plan things and actually the person is not that much interested in its completion. Before creating a plan to achieve some goals, we should note some small associated tasks and do it in parellel with creation of the plan. So the starting point of a plan should be set before actually finaliaing the plan.
