Fool me once shame on you, fool me twice shame on me. In all of my after action reports, I ask for what went well, what went wrong, what we can do better, and what we learned. Don't make the same mistakes over and over. Learn from them. Teach others and learn from others mistakes. There is no reason for everyone to trip over the same rock.
