To achieve massive wealth in life, you need to be in massive action. Many talks but less do. Execution is scarce and valuable. If you can think of something and execute it, you can do anything. EBM aims to improve patient care quality and produce optimal patient outcomes. It is identifying and promoting clinical practices that work, and eliminating those that don't. Additionally, the precision, accuracy and power of the intervention must be critically assessed. This process furthers the development of critical thinking.

Why use EBM?

According to AK Akobeng, there are five main steps to properly practise EBM. The next few blocks will explore this concept.

The 5-Step Model

Formulate Answerable Clinical Questions (CQs)The key is to translate the clinical problem into an answerable clinical question (CQ) – e.g. “Is treatment A effective in irradicating disease B?”. CQs should be clear, focus directly on the presenting complaint, and be answerable in terms of clinical evidence available.Form clinical questions using the PICO method.“P” - the Patient themself and the Problem i.e. their medical history and presenting complaint.“I” - Intervention i.e. the treatment / test used.“C” - Comparison of various interventions.“O” - Outcome i.e. the predicted result.

STEP 1

Find the Evidence : SourceLook for evidence that will answer your CQ. Clinical databases (e.g. Archimedes) are relatively reliable and timesaving sources, compared to traditional textbooks and oral sources. The Cochrane Database - a collection of system reviews - is also widely used in the medical field.

STEP 2A

Find the Evidence : SearchBeing able to search these databases is another skill in and of itself. Firstly, extract keywords from your PICO framework e.g. Problem is Thrombosis and Intervention is Aspirin, therefore the search will include the words “Thrombosis Aspirin”. When conducting the search, use the operating word “AND” for search results including all keywords and “OR” for either/some of the keywords.

STEP 2B

Appraise the EvidenceNot all research evidence is made the same – some is high-quality and thorough, some isn’t. It is thus paramount that research evidence is appraised in terms of validity, importance, and applicability. The CASP (Critical Appraisal Skills Programme) tools developed by Oxford Univ. incorporate all of these factors and provide a framework for your appraisal.

STEP 3

Apply the EvidenceEstablish the efficacy, risks, availability, and cost of the proposed treatment/test/intervention, and discuss with the patient before proceeding. This establishes trust between the health care practitioner and the patient - a "therapeutic alliance" as Akobeng says.

STEP 4

Evaluate PerformanceSelf-monitoring and -auditing are fundamental in EBM practice. Ask yourself, "Am I following all 5 steps to the best of my ability?". This ensures that you are always providing the best possible patient care, and that favourable patient outcomes are strived for and ultimately, achieved.

Cicero acknowledges that there are many good people we come in contact with in our lives we call our friends , be they business associates, neighbours, or any matter of  acquaintances. But he makes a key distinction between these common and quite useful friendships and those rare friends we bind ourselves to on a much deeper level. These special friendships are necessarily rare, because they require so much time and investment in ourselves. But these are the friends that deeply change our lives, just as we change theirs.

1. There are different kinds of friendships

People of poor moral character can have friends, but they can be only friends of utility for the simple reason that real friendship requires trust, wisdom, and basic goodness. Tyrants and scoundrels can use each other, just as they can use good people, but ba people can never find real friendship in life.

2. Only good people can be friends

We have to be deliberate about forming our friendships if for no other reason than that they can be very messy and painful to end if we found out the friends was not the person we thought.We should take our time, move slowly, and discover what lies deep in a person's heart before we make the investment of self that true friendship requires.

3. We should choose our friends with care

No one can thrive in isolation. Left on our own, we will stagnate and become unable to see ourselves as we are. A true friend will challenge you to become better because  he/she appreciates the potential inside you.

4. Friends make you a better person

No one is sweeter friend than someone who has been with you from the beginning. But don't limit yourself to the companions of youth, whose friendship may have been based on interests you no longer share. Always be open to new friendships, including those with younger people. Both you and they will be the richer for it.

5. Make new friends, but keep the old 

Friends will always tell you what you need to hear, not what you want them to say. There are plenty of people in the world who will flatter you for their own purposes, but only a real friend-or an enemy- will risk your anger by telling the truth. And being a good person yourself, you should listen to your friends and welcome what they have to say.

6. Friends are honest with each other

Cicero acknowledges that there are practical advantages to friendship- advice, companionship, support in difficult times-but at its heart true friendship is not a business relationship. It doesn't seek repayment, and it doesn't keep score.

7. The reward of friendship is friendship in itself 

A friend will risk much fo another, but not honor. If a friend asks you to lie, cheat, or do something shameful, consider carefully if that person is who you really thought he was. Since friendship is based on goodness, it cannot exist when evil is expected of it.

Friendships from youth will not be the same in old age-nor should they be. Life changes all of us with time, but the core values and qualities that drew us to friends in years past can survive the test of time. And like fine wine, the best of friendships will improve with age.
